What should be in a sitemap?
Only canonical, indexable pages that return 200 — no redirects, 404s, noindex pages or parameter URLs. Reference it from robots.txt with a Sitemap: line.
Does lastmod matter?
Only when it reflects real content changes. Setting every URL to the same or current date makes the field easy for search engines to ignore.
